First Visit

At your initial visit, we will gather all of your health and insurance information.  Dr. Moore will review your health history and perform an oral exam.  This exam will be focused on the area(s) of concern that your referring doctor has requested.  Dr. Moore will discuss the recommended treatment and answer any questions you may have.  If we require any additional information prior to treatment from other providers, we will gather this prior to your surgery appointment.

Finally, our staff will review your treatment plan with you and explain the fees for the recommended treatment and any insurance coverage you may have.  Please see our Financial Policy for details.  We will then schedule your surgery appointment if you are ready to proceed with the proposed treatment.

Please bring the following to your initial visit:

1. Your referral slip from your dentist, dental specialist, or physician.
2. Any xrays your referring doctor may have given you. If you do not have an xray, we will be happy to take any necessary xrays at our office.
3. Your list of medications (if applicable)
4. If you are under 18, you will need a parent or guardian with you for this appointment.
